Accessing someone else’s information

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

To requests proxy access, please collect a proxy access form from reception from 9am to 5pm.

Linked profiles in your NHS account

Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.

The NHS website has information about using linked profiles to access services for someone else.

Proxy Access for children 11-16 years old

From 11-16, a parent or guardian will need to be granted permission from the patient to access medical records and/or online services.

To apply for proxy access, the proxy must complete a Proxy Access Application Form available at the practice. or click here to download. 

Proof of the proxy’s identity and, if applicable, proof of parental responsibility or legal guardianship must be provided.

The patient (or parent/guardian if applicable) must sign the consent form.

For patients aged 11–15, the child’s written consent will be required if they are deemed competent to make decisions about their healthcare.
